Agreement and Notification Procedure
Key Card Issuance:
Each member is issued a personalized key card upon signing up for membership.
The key card provides 24-hour access to the salon facilities.
Responsibility:
Members are responsible for keeping their key card safe and secure.
The key card is non-transferable and should not be shared with others.
Lost or Stolen Key Card:
Members must notify Beach Booty Tanning Salon immediately if their key card is lost or stolen.
Notification can be made by calling the salon's customer service line or by visiting the salon during staffed hours.
Upon notification, the key card will be deactivated to prevent unauthorized access.
Replacement Key Card:
A replacement key card will be issued to the member for a nominal fee.
The member must provide identification and verify their membership details to receive a new key card.
Unauthorized Use:
If a lost or stolen key card is used fraudulently, the member must report any unauthorized transactions or access immediately.
Beach Booty Tanning Salon reserves the right to review security footage and take appropriate action if misuse is detected.
Acknowledgment:
By accepting the key card, the member agrees to these terms and acknowledges their responsibility to report a missing key card promptly.
